Please read the following procedure carefully when registering for a Fondy Phoenix 2010-2011 team.

• Players will be notified of their status by email, after the last tryout for a team.
• When a player accepts a position on a team, they have to reply to the email stating they accept.
• Complete attached registration form and attach it to email reply.
• Payment is to be mailed into the PO Box.
• All forms are due at the scheduled team meeting date, see Team Meeting page, Team meetings will be posted on this page as soon as they are set by the team rep/manager.
• New players should also bring a copy of their Birth Certificate or a copy of their passport to the team meeting.
• All players have to bring or send (only if registering after your team meeting) a passport size/small PICTURE for their player passes. Please write NAME (first and last) and Team (Gender, Age and Color) on the back of the picture
• If you are absolutely unable to attend the mandatory team meeting, you have to make arrangements with your team manager or coach, before the team meeting to drop off the forms etc.
• All Fondy Phoenix players have to order a uniform this year. Uniforms are different than the 2010 season.
